Currently, you are only able to check coverage for a brace with a maximum retail price of $200.00. For a brace that costs more than $200.00, please send an eligibility inquiry. Please indicate the total cost of the brace, the quantity, where on the body the brace will be worn, the medical diagnosis that requires the use of a brace and if the brace is custom-made. Please also indicate if the brace is required due to a motor vehicle accident, a work-related injury or if it’s being used for sports purposes only as that might impact eligibility.
Note: A custom brace, if included in your benefit plan, requires specific medical criteria to be met. An ‘Authorization Form for Custom Braces’ would need to be completed by your Medical Doctor and submitted to us for assessment.

Once the authorization form has been assessed, a written response outlining the amount that might be covered by your plan will be provided.
