An anterior mandibular positioner is a manufactured oral appliance that treats moderate to severe Obstructive Sleep Apnea (OSA) or central sleep apnea (CSA). It can be used in place of a CPAP.
Your plan may include this benefit, but eligibility for coverage requires an authorization form completed by a medical doctor or respirologist and a copy of a sleep study. The diagnosis must indicate moderate to severe OSA with an AHI reading of 15 or higher (the Apnea-Hypopnea Index reading measures the severity of sleep apnea by indicating the number of apneas and hypopneas experienced per hour of sleep).
The required form is the Authorization Form for Prosthetic Appliances and Durable Medical Equipment. To create a personalized form, visit the View personalized claim forms article.
Once the authorization form has been assessed, you will receive a written response outlining the amount that may be covered by your plan.
