In Alberta, New Brunswick, and Quebec
Hearing aids must be a benefit of your plan and prescribed by an Audiologist, Medical Doctor, Otolaryngologist (ENT specialist) to be considered eligible.
You'll need to submit a prescription by a valid prescriber (with their credentials) and an audiogram or a completed Hearing Aid Claim Form, and an audiogram including the prescriber’s information along with your claim.
In provinces where hearing aids are regulated (Alberta, New Brunswick, Ontario, or Quebec), hearing aids must be dispensed by an Audiologist or a Hearing Instrument Practitioner and you must include credentials, license information, and/or association information on the claim.
An audiogram must be submitted with all claims and include the name of the practitioner that performed the audiogram.

Ontario
If hearing aids are a benefit of your plan, they must be prescribed by an Audiologist, Medical Doctor, Otolaryngologist (ENT specialist) to be considered eligible for coverage.

The invoice must indicate if the Assistive Devices Program (ADP) is paying towards the claim. It must show $500.00 if ADP is paying toward the claim or $0.00 if they are not. Please speak with your hearing aid provider for more information regarding ADP.
